慢性前列腺炎患者前列腺液病原菌的菌种分布及其耐药性分析

Analysis of the distribution and drug-resistance of pathogenic bacteria in chronic prostatitis

目的探讨慢性前列腺炎患者前列腺液中病原菌菌种的临床分布及耐药性,为临床合理治疗提供依据。方法收集2007年1月至2008年12月慢性前列腺炎患者的前列腺液标本进行培养、鉴定和药物敏感试验,并对所得结果进行统计分析。结果385例患者的前列腺液标本中,培养出革兰阳性菌55株,占73.3%,其中又以溶血葡萄球菌为主,检测出34株,占45.3%;在检测出的45株葡萄球菌中,MRS36株,占80%。革兰阴性菌20株,占26.7%,其中又以大肠埃希菌为主,检测出9株,占12%;在15株大肠埃希菌和肺炎克雷伯菌中,检测出产ESBLs株3株,大肠埃希菌产酶率为22.2%。革兰阳性球菌对青霉素G的耐药率达到90%以上,对万古霉素敏感,未发现耐药株;革兰阴性杆菌对氨苄西林的耐药率达85%以上,对美洛培南和亚胺培南敏感,耐药率低于10%。结论对细菌性前列腺炎的治疗关键是在正确的病原学诊断和药物敏感试验指导下合理使用抗菌素。

Objective To study the distribution and drug-resistance of pathogenic bacteria in chronic prostatitis, so as to provide basis for reasonable use of drug in clinical practice. Methods Collect the specimens of pathogenic bacteria in chronic prostatitis in January 2007 to Decemeber 2008 for isolated,identified and antibiotic susceptibility test was done, the results were analyzed slatistically. Results In 385 strains specimens of EPS, there were 55 strains of gram-positive coccus, it＇ s about 73. 3 %, Staphylococcus haemolyticus was the most predominant accounted for 34 strains were 45.3% ;the MRS were 36 strains in the Staphylococcus haemolyticus, it＇ s about 80%. Gram-negative bacteris were 20 strians ,it＇ s about 26.7 % , and Escherichia. coli was the most, it were 9 strains about 12%. The ESBLs were 3 strains in the 15 strains of Escherichia. coli and Klebsiella pneumoniae. The Enzyme production rate of Escherichia. coli was 22. 2%. The resistant rate of Gram-positive cocci to Penicillin G was high than 90% ,and it was sensitive to vancomic, there was no resistant strain. The resistant rate of Gram-negative bacilli to Ampicillin was high than 85 %, and it was sensitive to Meropenem and imipenem, the resistance rate was low than 10%. Conclusion The key to the treatment of chronic bacterialprostatitis is the proper employment of antibiotics under the correct guidance of aetiological diagnosis and drug-sensitive experiment.
