摘要
目的:探讨脾脏在肝大部分切除后对其再生能力的影响。方法:将50只SD大白鼠随机分成两组。A组:行脾切除加70%肝大部分切除;B组:模拟脾切除加70%肝大部分切除。结果:在肝大部分切除后第1天和第2天,A组肝再生指数分别为2.63%±0.29%和3.51%±0.22%,B组分别为1.65%±0.27%和2.62%±0.32%,A组明显重于B组(P<0.05)。血清转氨酶:B组术后第1天972.47±189.25μmol/L,明显高于A组107.05±57.89μmol/L(P<0.005)。肝细胞核有丝分裂指数及肝细胞DNA增殖抗原指数术后第1天A组分别为39.7/1000±7.6/1000肝细胞、17.2%±0.54%;B组分别为9.84/1000±1.2/1000肝细胞、5.2%±0.23%(P<0.05和P<0.001)。结论:脾脏在肝大部分切除后,对肝再生起着十分重要作用。可能是脾脏在肝大部分切除后介导炎性反应而抑制肝脏再生。
Objective:To evaluate the role of spleen in hepatic regeneration after partial hepatectomy.Methods:50 female SD rats were divided at random into two groups:group A consisted of rats undergone splenectomy and partial hepatectomy;group B consisted of rats undergone shamed operation and partial hepatectomy.Results:24 and 48 hours after partial hepatectomy the weights of the liver in group A(2.63%±0.29% and 3.51%± 0.22 %) were significantly heavier than those in group B(1.65%±0.27% and 2.62%±0.32%)( P <0.05).The ALT of group B(972.47±189.25 μmol/L) 24 hours after operation was significantly higher than that of group A ( 107.05 ±57.89 μmol/L)( P <0.005).The mitotic index and DNA proliferative antigen index of hepatic cells in groups A (39.7/1 000±7.6/1 000 hepatic cells and 17.2%±0.54%) 48 hours after operation were significantly higher than those of group B (9.84/1 000±1.2/1 000 hepatic cells and 5.2%±0.23%)( P < 0.05 , P <0.001).Conclusions:The results show that the spleen plays an inhibitory role in hepatic regeneration after partial hepatectomy.It is possible that spleen can induce an inflammatory damage influencing hepatic regeneration after partial hepatectomy in rats.
