中等分化肝细胞癌形态定量与肿瘤预后及病理特征的关系

RALATIONSHIP BETWEEN THE PROGNOSIS AND PATHOLOGICAL FEATURES AND MORPHOMETRIC MEASUREMENT OF THE MILD DIFFERENTIATED HEPATOCELLULAR CARCINOMA

研究中等分化肝细胞癌癌细胞形态参数变化和肝癌患者预后以及肝癌生物学行为的关系。方法应用ＭＩＡＳ－３００自动图像分析仪对３８例中等分化肝癌癌细胞进行形态参数的定量分析。结果预后较好的肝癌患者的癌细胞细胞面积、周长、细胞核面积、核周长等参数值明显大于预后较差者；肝癌瘤体较大，癌细胞胞体和细胞核面积较小者具有易复发和多伴有肝硬化的趋势。肝癌预后与细胞和细胞核的形状因子无相关性。结论常规组织病理学诊断结合细胞形态定量分析有可能为临床患者预后评估等提供一些更加能够反映肿瘤细胞生物学特性的有价值的信息。

PURPOSE To investigate the ralationship of the morphometric features of the human hepatocellular carcinoma (HCC) and their prognosis and other clinical biological behavior. METHODS The morphometic measurement of the cell/nuclear area (CA/NA), cell/nuclear perimeter (CP/NP), cell/nuclear form factor (CFF/NFF) was performed in 38 cases of the mild differentiated human HCC. The relationship of the morphometric analysis and the clinical biological features such as HCC patient prognosis was analyzed. RESULTS The value of CA, CP, NA and NP of the relative longer survial HCCs are significant higher than those of the poor prognostic HCC cases; the HCC which have smaller value of CA, NA mostly reoccured and developed with liver cirrhosis. CONCLUSIONS The application of morphometric analysis on the basis of routine histopathological diagnosis might be valuable to present much more objective and accurate references about the biological features for the treatment of tumour and the evaluating tumor prognosis.
