烟雾病的临床特点及DSA表现

Clinical Features and DSA Appearances of Moyamoya Disease

目的探讨烟雾病(moyamoya disease,MMD)的临床特点及DSA表现。资料与方法15例患者经DSA诊断为MMD,分析其临床特点及DSA表现,并与CT资料对比分析。结果CT示脑出血11例(其中原发脑室出血6例),脑梗死3例,1例正常;DSA示双侧病变14例,单侧病变1例。所有病例均见颅底及基底节区异常增生血管网及颈内、外动脉侧支循环建立。结论对于反复出现脑缺血症状或CT提示原发性脑室出血、脑叶或颅底多发出血或梗死灶的中青年患者应考虑MMD可能,DSA可确诊MMD,并对侧支循环的评价具有独特的优势。

Objective To discuss the clinical features and the DSA manifestation of moya-moya disease.Materials and Methods The clinical features and the DSA manifestation of 15 patients with moya-moya disease were reviewed retrospectively.Results CT showed 11 cases with cerebral hemorrhage,of which 6 cases had primary intraventricular hemorrhage.3 cases had cerebral infarction,1 case was normal.On DSA,there were 14 cases with bilateral carotid artery stenosis or occlusion,1 case had unilateral lesion.Abnormal vessel network in the basal ganglion,collateral circulation vessels between ICA and ECA were found in all cases.Conclusion If cerebral ischemia are suffered frequently,and primary hemorrhage in ventricle,hemorrhage or infarction in basal ganglion or intracranial are detected by CT in young man,moya-moya disease should be considered.The DSA could not only define the diagnosis,but also has a unique advantage to evaluate collateral circulation.
