CIN和宫颈癌患者手术前后血清及病变组织中PGE2水平变化及意义

Significance of prostaglandin E2 level changes in tissue and perioperative serum of patients with cervical intraepithelial neoplasia and cervical cancer

目的:测定宫颈上皮内瘤样病变(CIN)及宫颈癌患者手术前后外周血清及病变组织中前列腺素E2(PGE2)含量并探讨其临床意义.方法:采用酶联免疫吸附法(ELISA)分别测定47例病例组患者(CIN14例,宫颈癌33例)手术前后外周血清及病变组织以及正常对照组20例标本中(健康献血女性血清20例及因子宫肌瘤行全子宫切除术患者的正常宫颈20例)PGE2含量.结果:宫颈癌患者术前血清及癌组织中PGE2水平明显高于CIN组及正常对照组(P<0.05);宫颈癌及CIN组术后血清FGE2水平较术前下降显著(P<0.05);宫颈癌组织及外周血中PGE2含量与肿瘤直径、FIGO临床分期及淋巴结转移与否有显著差异(P<0.05),而与患者年龄、病理组织学类型及分化程度无关(P>0.05);病例组术前血清与术后血清中PGE2含量呈正相关(r=0.961,P<0.001);术前、术后血清中PGE2含量均与宫颈组织PGE2含量呈正相关(r分别为0.859和0.838,P<0.001).结论:外周血PGE2水平与癌组织释放的PGE2含量成正比.检测外周血PGE2水平对早期发现CIN向宫颈癌转化及监测宫颈癌的淋巴结转移具有一定价值.

AIM：To observe the prostaglandin E2（PGE2）levels in tissue and changes of serum between pre-and post-operative patients with cervical intraepithelial neoplasia（CIN）and cervical cancer and to evaluate its clinical significance.METHODS：Pre-and post-operative serum and tissue samples of 47 patients with CIN（14 cases）or cervical cancer（33 cases）were collected.Serum samples from 20 health women and cervix tissue samples of 20 patients undertaken total hysterectomy due to myoma were collected for the control.PGE2 levels were measured by means of ELISA.RESULTS：The PGE2 levels in preoperative serum and tissues of cervical cancer were higher than that of CIN and normal control group（P〈0.05）,and PGE2 in CIN group only in preoperative serum were higher than control group（P〈0.05）.The serum PGE2 levels of postoperative in CIN and cervical cancer patients were significantly lower than that of preoperative（P〈0.05）.The PGE2 levels of peripheral blood and tissue of cervical cancer patients were statistically different for the tumor size,FIGO stages and lymph node involvement（P〈0.05）.There were no significant relationship between age,patho-histologic type and differentiation degree.The PGE2 levels in preoperative serum positively correlated to that in postoperative serum of patients with CIN and cervical cancer（r=0.961,P〈0.01）,and the PGE2 levels both in preoperative and postoperative serum correlated to that in tissue of case group（r were respectively 0.859 and 0.838,P〈0.001）.CONCLUSION：PGE2 levels of perioperative blood are proportional to the contents of cancer tissue released.Measurement of PGE2 might be helpful to detect transform from CIN to cancer and pelvic lymph node metastasis.Therefore,PGE2 could be a potential diagnostic index for cervical cancer.
