复方芎芷散治疗血管性头痛临床疗效观察

Clinical Study of Compound Mixture of ChuanXiong and BaiZhi's Treatment Results on Vascular Headache of Blood Stagnation Type

目的:观测复方芎芷散对瘀血阻络型血管性头痛临床治疗效果。方法:2007年1月—2009年5月上海市第七人民医院中医科和神经科90例诊为瘀血阻络型血管性头痛患者,经超声多普勒检测均有脑血管痉挛,血流速度改变。随机分为3组,分别为单纯中药组(A组)、中西医结合组(B组)、单纯西药组(C组),治疗3个月后观测治疗效果,进行统计学处理。结果:90例全部进入结果分析,①治疗过程中各组患者均未出现不良反应,试验后患者血、肝肾功能、尿常规均未出现明显异常。②A组治愈20例,显效7例,总有效率90.00%;B组治愈23例,显效5例,无效2例,总有效率93.33%;C组治愈18例,显效5例,无效7例,有效率76.67%。A组与B组的疗效均优于C组,差异有显著性(P<0.05);A组与B组比较疗效无显著性差异。③治疗前,3组之间全血高切与低切黏度、血浆黏度无显著性差异;治疗后,A组、B组全血高切与低切黏度、血浆黏度与治疗前比较,均明显下降(P<0.05);治疗后,A组、B组之间全血高切与低切黏度、血浆黏度无显著性差异,C组治疗前后全血高切与低切黏度、血浆黏度无显著性差异。结论:复方芎芷散与西比灵胶囊合用有协同作用,对瘀血阻络型血管性头痛有较好的治疗作用,主要机理与改善血液流变学状态、抗增殖、缓解平滑肌的紧张状态,改善瘀血的病理状态,从而对属于瘀血阻络的偏头痛取得较好疗效。

Objective： Clinical observation on the therapeutic effect of Compound Mixture of ChuanXiong and BaiZhi＇s treatment results on vascular headache of blood stagnation type .Methods： From january 2007--May 2009,90 outpatients, with blood stagnation type vascular headache from TCM and neurology departments in our hospital. Divided into three groups randomly： Chinese medicine group （ A group ） scombined group of TCM and western medicine （ B group ） swestem medicine group （ C group ）, clinical observation on the therapeutic effect after 3 months, then statistically treat the resuhs. Results： All of the 90 patients were included in the results analysis, （1） No adverse reaction of all the patients , liver and kidney function, blood and urine test before and after treatment are normalo （2）Group A ： 20 patients cured, 7 are effectual, total effectual rate is 90.00% ; Group B ： 23 patients cured, 5 are effectual, total effectual rate is 93.33%; Group C： 18 patients cured,5 are effectual, total effectual rate is 76.67%0 Effect of group A and B are better than group C, statistic difference is obvious （P〈0.05）; Comparison between group A and B , statistic differenc is not obvious. （3） Before the treatment, statistic difference of plasma viscosity on the three groups is not obvious ; After the study, compared with before the treatment, plasma viscosity of group A and B are obviously decreased （10〈0.05）; Compared the plasma viscosity between group A and group B, statistic difference is not obvious. About group C＇s plasma viscosity of before and after study, statistic difference is not obvious too. Conclusion ： Compound Mixture of ChuanXiong and BaiZhi have synergistic effect with flunarizine on the treatment of blood stagnation type vascular headache. The main mechanism may is it can improve the hemorheology condition of this type of patients.
