慢性乙肝不同临床类型患者CD_4^+、CD_8^+细胞水平分析

Analysis of the levels of CD_4^+ and CD_8^+ lymphocytes in the patients with different clinical types of chronic hepattits B

目的了解慢性乙型肝炎不同临床类型外周血中的CD4+、CD8+细胞水平。方法以慢性乙肝患者作为研究对象采集其外周血,采用流式细胞仪分别对研究对象外周全血进行淋巴细胞亚群CD4+和CD8+检测。应用SPSS 13.0For Window统计软件对3组不同临床类型研究对象的CD4+和CD8+细胞水平进行两两比较。结果慢重肝组与肝硬化组CD4+和CD8+细胞水平比较的均差、标准误、P值及95%可信区间分别为:-0.018,0.029,0.546,-0.076~0.041和0.072,0.020,0.000,0.033~0.112;慢重肝组与慢乙肝组CD4+和CD8+细胞水平比较的均差、标准误、P值及95%可信区间分别为:0.024,0.027,0.391,-0.078~0.031和0.016,0.019,0.405,-0.021~0.052;肝硬化组与慢乙肝组CD4+和CD8+细胞水平比较的均差、标准误、P值及95%可信区间分别为:-0.006,0.030,0.844,-0.065~0.053和-0.057,0.020,0.006,-0.097^-0.017。结论3组病例CD4+水平均无显著性差别,提示CD4+细胞对慢性乙肝的临床类型影响不大。3组间CD8+水平相互比较在慢重肝组与慢肝组无显著性差别,但在慢重肝组与肝硬化组、肝硬化组与慢肝组比较均存在显著性差异,提示CD8+细胞可影响慢性乙肝的临床类型。

Objective To analyse the levels of CD4^＋ and CD8^＋ lymphocytes in the patients with different clinical types of chronic hepattits B. Methods The peripheral blood was collected from the patients with different clinical types of chronic hepatitis B. The levels of CD4^＋ and CD8^＋ lymphocytes in the peripheral blood from the subjects were examined by flow cytoanalyze model （EPICS XL） .Results The results showed that the mean difference, standard error, p value and the 95% confidence interval were -0.018, 0.029, 0.546, -0.076- 0. 041 and 0. 072, 0. 020, 0. 000, 033 - 0.112 respectively in comparison of the CD4^＋ and CD8^＋ lymphocyte level between the severe hepatitis B （SH） and the cirrohsis （LC） groups. The mean difference, standard error, p value and the 95 % confidence interval were 0. 024, 0. 027, 0. 391, - 0. 078 - 0.031 and 0. 016, 0. 019, 0. 405, - 0. 021 - 0. 052 respectively in comparison of the CD4^＋ and CD8^＋ lymphocyte level between the SH and the chronic hepatitis B （CB） groups. The mean difference, standard error, p value and the 95% confidence interval were - 0. 006, 0. 030, 0. 844, - 0. 065 - 0. 053 and - 0. 057, 0. 020, 0. 006, - 0. 097 - - 0. 017 respectively in comparison of the CD4^＋ and CD8^＋ lymphocyte level between the LC and the CB groups. Conclusions No significant difference in comparison of the mean difference level among the three groups suggests that CD4^＋ lymphocyte might play no important role in the development of the different clinical types of chronic hepattits B. Significant difference of CD8^＋ lymphocyte level is in comparison of the SH and the LC groups, the LC and the CB groups, suggests that CD8^＋ lymphocyte might play a part role in the development of different clinical types of chronic hepattits B.
