摘要 目的分析湖北省艾滋病主要传播模式的变化情况,为调整艾滋病防治策略提供依据。方法收集、整理、分析1988-2008年湖北省艾滋病病例报告,国家血清学哨点及综合监测点资料、全省主动监测资料等。结果1988-2008年,湖北省新发现的艾滋病病毒(HIV)感染人数明显上升,最近5年保持在600-800之间。从2002年开始感染途径构成发生明显改变,经性途径感染逐年上升,由2002年9.24%升至2008年63.67%,而同期经血途径感染由79.62%下降为22.41%,每年发现的静脉吸毒感染者数及该人群HIV感染率变化不大。HIV感染者主要聚集在该省西北部和东部经济不发达地区以及人口流动性大的地区。结论湖北省艾滋病经血传播逐渐下降,经性传播已经成为主导模式,异性性途径感染和同性性途径感染同时增加。 Objective To analyze changes of HIV/AIDS transmission model in Hubei province so as to provide evidence for policy adjustment for AIDS prevention and control. Method AIDS case reports and data from the national serological sentinel sites and from behavioral surveillance sites, as well as data from active surveillance between 1988 and 2008 in Hubei province, were collected and analyzed. Results New cases of HIV infections were rising obviously from 1988 to 2008, and was maintained between 600 to 800 in the last five years. Since 2002, HIV/ AIDS transmission routes had been obviously changed. The number of infections via sexual transmission was increased, from 9. 24% in 2002 to 63.67% in 2008. And during the same period, infections via blood transmission dropped from 79.62% to 22.41%. Comparatively, less change was seen in the number of HIV cases infected via intravenous drug and so was the infection rate. The cases of HIV infection were concentrated in the underdeveloped northwestern and eastern part of Hubei province, and also in areas with high population mobility. Conclusion The number of HIV cases transmitted via blood transfusion has gradually decreased in this province, and infection via sex has become the major route of transmission with homosexual and heterosexual risk behaviors increasing simultaneously.
