重视高血糖高渗状态合并脏器衰竭的早期诊断与救治

Attention to Early Diagnosis and Treatment of Hyperglycemic Hyperosmolar Combined with Organ Failure

目的探讨高血糖高渗状态合并脏器衰竭的早期诊断和治疗对患者预后的影响。方法收集2003年5月至2008年3月在鹤煤(集团)公司第二职工医院抢救治疗的13例高血糖高渗状态合并脏器衰竭患者,对患者的血糖、尿糖、尿素氮、血钠、血钾、血浆渗透压、二氧化碳结合力等重要指标进行监测,结合脏器衰竭的数目,对比患者死亡例数的差异。结果13例中并发2种脏器衰竭者7例均全部抢救成功;并发3种脏器衰竭者有2例,全部死亡;并发4种脏器衰竭者4例,全部死亡。结论高血糖高渗状态并发症多而且严重,病死率高,早期诊断和正确治疗具有重要意义。

Objective To discuss the early diagnosis and treatment to patients with combination of hyperglycemic hyperosmolar and organ failure.Methods 13 patients with combination of hyperglycemic hyperosmolar and organ failure from May 2003 to March 2008 in the second hospital of Hebi Coal Group company were collected.The key indicators such as blood sugar,urine sugar,blood urea nitrogen,serum sodium,serum potassium,plasma osmotic pressure,carbon dioxide binding were monitored,combined with the number of organ failure,compared to the number differences of death cases.Results During 13 cases,7 cases of two organ failure complications were successfully rescued;2 cases of three organ failure were dead,4 cases of four organ failure were dead.Conclusion The complications of hyperglycemic hyperosmolar were serious and had high mortality rate,the early diagnosis and proper treatment were important.
