骨质疏松椎体压缩性骨折经皮椎体成形术与保守治疗对照临床研究

Osteoporotic vertebral compression fractures-Percutaneous vertebroplasty versus conservative therapy

目的探讨经皮椎体成形术治疗老年骨质疏松性椎体压缩骨折的临床效果。方法自2007年8月至2008年9月采用经皮椎体成形术治疗老年骨质疏松性椎体压缩骨折患者25例,与同期保守治疗的17例患者进行对比研究。结果经皮椎体成形术组与保守治疗组相比,住院时间明显缩短,患者症状缓解较早,早期可开始下床活动,所有病例压缩椎体前缘高度恢复明显,经统计学分析差异有显著性,而后凸Cobb角矫正不明显,随访8～12个月(平均10个月)后加固椎体高度未见明显丢失。结论经皮椎体成形术是一种简单、微创、安全有效的治疗方法,但要熟练掌握其适应证。

Objective To investigate the clinical outcomes in the treatment of osteoporotic vertebral compression fractures with percutaneous vertebroplasty （PVP）. Methods The clinical results of a group of 25 patients, who had suffered from the osteoporotic vertebral compression fractures and treated with percutaneous vertebroplasty, were compared with those of another group of 17 patients treated by non-operative procedure. Results The duration of hospitalization, the time of pain remission and the out of bed activity were significantly reduced in the PVP group as compared with those undergone non-operative procedure. On following up from eight months to twelve months, the anterior height of injured vertebral body of all patients were generally recovered obviously, and the Cobb angles were not rectified. Conclusion The PVP is an effective procedure of simple microinjure, with less complications, wide indication and good outcome, and it is essential to master its indication meanwhile.
