老年股骨颈骨折不同治疗方法分析

ANALYSIS ON DIFFERENT TREATMENTS IN SENILE FEMORAL NECK FRACTURES

目的探讨空心加压螺纹钉固定、人工股骨头置换、人工全髋关节置换在不同患者中的应用价值。方法通过回顾性分析2000年1月—2002年12月收治的258例股骨颈骨折患者的临床资料,分别对手术情况、关节功能恢复情况、产生并发症情况进行临床随访分析,随访时间3～5年,平均4.5年。结果行空心加压螺纹钉固定68例,术后骨折不愈合2例,髋内翻1例,股骨头坏死9例;行人工股骨头置换121例,出现感染2例,松动2例,髋臼磨损7例;全髋置换组69例,出现松动1例,假体周围骨折1例。结论老年股骨颈骨折根据患者年龄、身体状况、骨折类型选用不同的治疗方法,疗效满意。

Objective To discuss the value of the hollow compression screw bolt fixation, the artificial femoral head replacement surgery, the articulation coxae replacement surgery in different patients. Methods The clinical data of 258 cases with femoral neck fracture who were treated from Jan 2000 to Dec 2002 were analysed. Clinical revisit was per-formed in surgery situation, situation of the joint function restoration and situation of the complication, the revisit duration was 4. 5 years （ 3 years - 5 years ）. Results 68 patients were treated with the hollow compression screw bolt. In which,2 patients were with fracture disunion, 1 patient with coax adducta, 9 patients with femoral head necrosis; 121 patients were treated with artificial femoral head replacement surgery. In which, 2 patients had infection,2 patients＇ artificial femoral head loosed ,7 patients had acetabulum attrition; 69 patients were treated with artificial entire thurl replacement surgery. In which,1 patient had phantom loosing , 1 patient had the fracture around phantom. Conclusion Aceordiing to patient＇s age, physical condition and fracture type, the treatment results of senile femoral neck fracture was satisfied.
