髋关节脱位合并髋臼骨折临床诊治分析

Analysis of the clinical diagnosis and treatment on the dislocation of the hip combined with acetabular fractures

目的:探讨髋关节脱位合并髋臼骨折的临床诊治方法。方法:我科2001年1月～2008年6月收治的30例髋关节脱位合并髋臼骨折患者中,8例采用保守治疗,在腰麻或硬膜外麻下行ALLis法或Bigeloo法整复,并摄前后位X线片检查复位情况;22例行手术治疗,均经CT检查,其中,7例因关节腔内游离骨片阻碍复位行急诊手术,余15例均在手法复位股骨髁上牵引3～7d后手术。结果:60例均得到随访,随访时间6个月～3年,平均1.5年。优9例,良13例,中5例,可2例,差1例,优良率为73.3%。结论:选择合适的方法治疗髋关节脱位合并髋臼骨折有助于髋关节功能恢复和避免并发症的发生。

Objective： To investigate the dislocation of the hip combined with acetabular fractures clinical diagnosis and treatment. Methods： 30 cases of hip dislocation in patients with acetabular fractures were selected from January 2001 to June 2008 treated in our hospital, 8 cases of conservative treatment, in the spinal anesthesia or hardware outside the law or Bigeloo Act ALLIS restoration, checked with X-ray to show the situation of reset. 22 cases with surgical treatment, they had been considered by the CT examination, and 7 cases of intra-articular sclerites hinder free reset emergent surgery, 15 cases were in the manual reduction of femoral supracondylar traction 3-7 days after the surgery. Results： 30 cases were followed up for 6 months to 3 years, with an average 1.5 years. 9 cases of excellent, good in 13 cases, 5 cases in medial level, 2 cases were just so and poor in 1 case, the excellent rate was 73.3%. Conclusion： The choice of appropriate methods of treatment of hip dislocation and acetabular fractures contribute to the restoration of hip function and to avoid the occurrence of complications.
