无创正压机械通气治疗COPD合并Ⅱ型呼吸衰竭疗效观察

Clinical effect observation of noninvasive positive pressure mechanic ventilation in treatment of chronic obstructive pulmonary disease(COPD)with typeⅡrespiratory failure

目的观察无创正压机械通气(NIPPV)治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的临床疗效。方法 32例COPD合并Ⅱ型呼吸衰竭患者随机分为通气组和对照组,对照组19例,给予吸氧、药物等常规治疗;通气组13例,给予常规治疗的同时加用NIPPV治疗。监测两组治疗前后血气指标。结果两组治疗前后血气指标均有改善,治疗组改善程度明显优于对照组,两组疗效比较有显著性差异(P>0.05)。结论 NIPPV治疗COPD合并Ⅱ型呼吸衰竭疗效显著。

Objective To observe clinical effect of noninvasive positive pressure mechanic ventilation （ NPPV ） in treatment of chronic obstructive pulmonary disease（COPD） with type Ⅱ respiratory failure. Methods Thirty - two patients with COPD with type Ⅱrespiratory failure were divided into the treatment group and the control group at random. The control group of 19 cases were given routine treatment （eg. Inhaling oxygen and medicine）. The treatment group of 13 cases were treated with noninvasive positive pressure mechanic ventilation in addition to above routine treatment. The blood gas analysis of two groups were observed before and after treatment. Results The blood gas analysis of two groups were both improved before and after treatment. The blood gas analysis changed significantly more in the treatment group than that in the control. There was significant difference on effect between two groups （ P 〉 0. 05 ）. Conclusion Noninvasive positive pressure mechanic ventilation（NPPV） in treatment of chronic obstructive pulmonary disease（COPD） with type Ⅱ respiratory failure has significant effect.
