缺血性脑血管动脉病溶栓介入治疗临床应用

The clinical application of ischemic cerebrovascular intraarterial thrombolytic therapy

目的探讨动脉介入溶栓治疗缺血性脑血管动脉病(ICVD)的临床应用。方法对34例ICVD患者的临床资料进行回顾性分析,并与常用的静脉溶栓药物治疗组进行对照。结果术后1年内再次发生中风的7例,动脉溶栓组2例;病情稳定的21例,其中静脉溶栓组5例,动脉溶栓占13例,其中2例动脉溶栓患者30 d后完全恢复正常生活,无神经缺陷;6例患者神经功能都有不同程度改善,生活能自理,动脉溶栓占4例。与静脉溶栓药物治疗组对照,再次中风发生率低,术后功能恢复好。结论动脉介入溶栓治疗急性脑血管疾病较静脉溶栓药物治疗可以更有效发挥药物局部作用,减少药物副作用对各系统影响,减轻大脑神经功能缺损。

Objective Discussing the clinical application of intraarterial thrombolytic therapy for acute cerebral vascular disease （ICVD）. Methods The clinical data of 34 cases of patients with ICVD were retrospectively analyzed and compared with the conventional intravenous thrombolytie treatment. Results There were 2 cases in arterial thrombolysis group of the 7 cases of relapser until one. There were 10 cases in arterial thrombolysis group of 19 cases of stable state of illness, T2 cases of patients with arterial thrombolysis fully returned to normal life with no nerve defects. Neurological assessment of 6 patients were improved. Compared with intravenous thrombolysis treatment control group, there was low incidence of relapse and good postoperative functional recovery. Conclusion Compared with intravenous thrombolysis treatment, intraarterial thrombolytic therapy for acute cerebral vascular disease can effectively reduce brain neurological deficit and further reduce the incidence of relapse.
