多囊卵巢综合征经腔三维能量多普勒超声参数及血管化指标特征的分析 被引量:8

Analysis of the ovarian characteristics of transintracavitary and vascularization index of the polycystic ovary syndrome

摘要 目的探讨多囊卵巢综合征(PCOS)经腔三维能量多普勒超声特征。方法运用二维超声、彩色多普勒血流显像及三维能量多普勒超声对75例PCOS患者、63例正常者进行观察,测量卵巢平均直径、最大切面卵泡数、搏动指数(PI)、阻力指数(RI)、卵巢容积(V总)、间质容积(V间质)、卵泡容积(V卵泡)、平均灰度值(MG)、血管化指数(VI)、血流指数(FI)和血管化一血流指数(VFI)。结果PCOS卵巢平均直径、卵泡数、V总、V间质、V卵泡、VI和VFI大于对照组,差异有统计学意义(P〈0.05或P〈0.01)。PCOS卵巢PI、R1小于对照组,差异有统计学意义(P〈0.01)。结论PCOS患者具有多囊卵巢超声影像学特征性的改变,三维能量多普勒超声可以精确测量卵巢容积,定量评估卵巢间质血流,VI和VFI可作为辅助诊断PCOS的新指标。 Objective To quantify the transintracavitary three dimensional power Doppler ultrasound features of ovaries in polycystic ovarian syndrome(PCOS). Methods Two dimensional ultrasound, color Doppler flow imaging, three dimensional power Doppler ultrasound was performed on 75 women with PCOS and in 63 controls. Such key index as ovarian average diameter, follicle count, pulsatility index(PI), resistance index(RI), total ovarian volume, stromal volume, follicle volume,mean gray value(MG) were measured and ovarian stromal blood flow index such as vascularization index(VI) ,flow index(FI) and vascularization flow index(VFI) were also quantified. Results In comparison with control group, patients with PCOS showed significantly higher ovarian average diameter, follicle count, total ovarian volume, stromal volume, follicle volume, VI and VFI, while PI and R I were lower. Conclusions Patients with PCOS are characterized by an ultrasound change of polycystic ovary. Three dimensional power Doppler ultrasound can not only measure the volume of ovary but also assess the blood perfusion of ovarian stroma. VI and VFI can serve as new criteria for the auxiliary diagnosis of PCOS.
