伊曲康唑和局部抗真菌药物(2%酮康唑洗剂)联合治疗花斑癣的临床效果观察

A clinical study to treat pityriasis versicolor with the united treatment of itraconazole and local meidi-cation that repel epiphytes

目的观察伊曲康唑(美扶)胶囊和局部抗真菌药物(2%酮康唑洗剂)联合治疗花斑癣的临床疗效和复发率。方法选择358例花斑癣患者,随机分成2组,实验组200例,对照组158例。两组均为皮损处外用药(2%酮康唑洗剂),2～3次/d,每周门诊复查1次。实验组加用口服伊曲康唑胶囊200mg,2次/d,共7d,餐中或餐后立即服用,停药3w为一疗程。记录治疗效果及观察不良反应,连续4w后判定结果。结果试验组200例,痊愈182例(91%),显效11例(5.5%),有效6例(3%),复发1例(0.5%),总有效率193例(96.5%);对照组158例,痊愈52例(32.91%),显效23例(14.56%),有效54例(34.18%),无效6例(3.80%),总有效率75例(47.47)%,复发23例(14.55%)。两组总有效率、复发率比较,差异均具有极显著性(χ2=112.758,P<0.005;χ2=27.887,P<0.005)。结论伊曲康唑(美扶)和局部抗真菌药物(2%酮康唑洗剂)联合治疗花斑癣,能提高花斑癣治疗的总有效率,同时降低复发率。

Objective To observe the clinical effect and recurrence rate to treat pityriasis versicolor with the united treatment of itraconazole and the local medication that repel epiphytes.Methods 358 patients with pityriasis versicolor were selected and divided into two groups at random.There were 200 patients in the trial group and 158 patients in the control group.All patients took a part drug against of epiphytes,2 or 3 times per day.The patients in the trial group orally took itraconazole 0.2g twice a day for 7 days.The thera-peutic effects were evaluated once a week for 4 weeks.Results The clinical cure rate and recurrence rate in the trial group was 96.5% and 0.5%,while it was 47.47% and 14.55% in the control group.There were statis-tically significant differences between the two groups（χ2=112.758,P〈0.005;χ2=27.887,P〈0.005）.Conclu-sion The united treatment of itraconazloe and the part drug against epiphytes in pityriasis versicolor can re-duce the recurrence rate and increase the cure rate.
