脑干听觉诱发电位在后颅凹肿瘤诊断及术中监护的应用

摘要 目的:了解脑干听觉诱发电位技术在后颅凹肿瘤诊断及术中监护的意义。方法:对8例后颅凹肿瘤患者进行术前检查,其中2例进行术中监护,对BAEP各波波形、潜伏期、波间期的变化进行分析。结果:Ⅰ、Ⅲ、Ⅴ波潜伏期延长4例;ⅠⅢ、ⅢⅤ或ⅠⅤ波波间期延长5例;波形分化不良,重复性差6例;由于手术操作引起的颅内压变化、脑干出血、缺血、受压均可引起BAEP的变化。结论:BAEP对后颅凹病变的早期诊断。 Objectives:To study the significance of brainstem auditory evoked potential (BAEP) changes in diagnosis and intraoperative monitoring of postcranial fossa tumor. Methods:BAEPs were performed preoperatively in 8 patients, 2 of them were monitored during operations. The wave forms, the latencies (PL) and interpeak latencies (IPL) were analysed. Results:The PL of Ⅰ、Ⅲ、Ⅴ waves prolonged in 4 patients, and the IPL of ⅠⅢ,ⅢⅤ or ⅠⅤ prolonged in 5 patients. The waveforms undiffentiated in 6 patients. Intraoperative monitoring with BAEP showed that the BAEP changes could be influenced by changes of intracranial pressure,brainstem bleeding and ischemia or compression due to procedure. Conclusions:BAEPs might be of great significance in early diagnosis of postcranial fossa tumor, intraoperative monitoring and the evaluation of the brainstem function postoperatively. 
