穿透性角膜移植治疗圆锥角膜的疗效观察

An Ivestigation on Therapeutic Effects of Penetrating Keratoplasty for Treatment of Keratoconus

目的探讨穿透性角膜移植治疗圆锥角膜的疗效。方法对12例15眼圆锥角膜行穿透性角膜移植．其中男9例，女3例，男女之比为3：1。年龄最小12岁，最大32岁，平均19．2岁。手术操作按常规的角膜移植手术进行。植片均从内皮面压切取得，直径与植床等大，选择环钻直径以切除圆锥底为准。手术以10－0尼龙线连续缝合．术后拆线时间最短8个月，最长12个月，平均11个月。结果术后随访10个月～3年，植片全部透明，透明率达100％。病情发展得到了有效控制。1例发生内皮排斥反应，排斥反应率为6.8％。术后矫正视力≥0.4者12只眼．占80％。结论穿透性角膜移植可明显改善圆锥角膜特别是已有瘢痕形成者的患者的视力，且并发症少．是目前治疗该病的最佳选择。

Objective To study therapeutic effects of Penetrating keratoplasty for treatment of keratocous. Methods 12 cases (15 eyes) of keratoconus were performed penetrating keratoplasty. 9 cases were male and 3 female. The age ranged from 12 to 32 years old, an average age of 19. 2 years. the surgical technique is described. Results After a follow-up of 10 months to 3 years. The disease was effectively controlled. The rate of graft transparency was 100%, and the immune rejection rate was 6. 8% (1 eye).Corrected vision was 0. 4 or better in 12 eyes after the operation (80% ). Conclusions keratoplasty is the best choice for treatment of keratoconus.
