经脐单孔腹腔镜肾切除术:首例经验与体会

Transumbilical one-trocar laparoscopic nephrectomy:the first clinical report

目的汇报国内首例经脐单孔腹腔镜肾切除术的经验与体会,并探讨其可行性。方法1例左肾萎缩无功能合并同侧肾结石患者行单孔腹腔镜肾切除术。在脐部做一4cm切口,经脐建立腹腔镜单孔4通道操作通道,维持气腹并完成肾切除手术。结果手术时间为126min,手术顺利完成,切除左侧肾脏同时取出结石,术后未放置引流管,无出血等并发症发生,术后第3天患者顺利出院,脐部几乎无明显手术瘢痕。结论经脐单孔腹腔镜肾切除术安全可行,较传统多孔腹腔镜更加微创美容。

[Objective] To report the initial clinical experience of nephrectomy using transumbilical one-trocar laparoscopic technique. [Methods] A patient with non-functioning left kidney combined with stone underwent transumbilical laparoscopic nephreetomy using the laparoscopie single-site surgery procedure with a quad-channel access port（R-Port）. The complications and technical issues during the procedure were noted. [ Results ] The transumbilical one-trocar laparoscopic nephrectomy was performed successfully. The operative time was 126 minutes. No drainage tube was placed, and there were no intraoperative or postoperative complications. The patient was discharged 3 days after operation with the scar receded into the umbilicus and hardly visible. [ Conclusions] The tran- sumbilical one-trocar laparoscopic nephrectomy is safe and feasible. The procedure has obvious cosmetic and minimolly invasive advantages compared to traditional laparoscopy.
