维持性腹膜透析患者营养状态的研究 被引量:7

摘要 目的采用前瞻性临床研究设计,探讨我院腹膜透析患者营养状况及营养不良的发生率和原因。方法①血生化检查:血清白蛋白(ALB)、血红蛋白(HGB)、转铁蛋白(TF)。②蛋白摄入和代谢:nPNA。③人体指数学测量:测定肱二头肌皮褶(BSF)、肱三头肌皮褶(TSF)、上臂中部周径(MAC),MAMC、AMA、BMI测量。④整体主观营养状况评估:使用SGA评估法进行评估。结果①腹膜透析患者营养不良发生率为70.2%,其中轻中度营养不良为62.3%,重度营养不良为7.9%。②人体指数学:BMI:(20.3±3.2);BSF:(7.1±4.1)mm;③血生化:ALB:(33.5±5.2)g/L,Hb:(86.2±12)g/L,TF:(1.6±0.8)g/L。④nPNA:(0.65±0.10)g(/kg·d)。⑤SGA评分:(4.2±1.1)分。结论①腹膜透析患者营养不良发生率为70.2%,与国外同类患者相比,我们的透析患者营养状况较差。②人体指数学是一种简单有效的评估人体成分、肌肉营养和体蛋白的方法。③SGA整体营养评估方法能反映腹透患者营养状况,无创易于操作,值得首选。
