摘要
目的探讨鼻腔粘连对鼻窦内窥镜手术疗效的影响。方法应用Stryker硬性鼻内镜,根据不同的病变部位,参照Messerklinger鼻内镜手术方法,行鼻内镜手术378例(640侧),术后定期随访1年以上。结果治愈506侧,总治愈率79.1%。其中I型病例治愈率达91%以上;Ⅱ型1期和2期分别为98.4%和84.7%,3期为75%;Ⅲ型治愈率为62.7%。在134侧无效病例中鼻腔粘连115侧,占85.8%。结论鼻内镜术后鼻腔粘连是影响手术疗效的主要原因,减少术后局部粘连是提高手术疗效的关键。
Objective To observe a effect of postoperative nasal cavity adhesion on the long-term results of endoscopic sinus surgery.Method 378 patients （640 sides） with sinusitis and polyps were operated using endoscopic sinus surgery according to Messerklinger.These patients were follow-up for 1 year. Result 506 sides were cured,total cure rate was 79. l%.The cure rates of type Ⅰ,type Ⅱ stage 1,type Ⅱ stage 2,type Ⅱ stage 3 and typeⅢ patients were 91%,98.4%,84.7%,75% and 62.7% respectively. In 134 sides failure, 115 sides had nasal cavity adhesion, 85.8%. Conclusion The postoperative nasal cavity adhesion was the main factor which can cause the failure of the endoscopic sinus surgery.The middle tubinate played an important role in preventing nasal cavity adhesion
