胰岛细胞移植治疗成人Ⅰ型糖尿病的临床应用初探

摘要 目的初步探讨成人胰岛细胞移植治疗成人Ⅰ型糖尿病的临床应用价值.方法自2006年开始经不断探索和反复的动物实验,初步掌握了成人胰岛细胞移植技术,并应用于1例临床I型糖尿病伴严重肠系膜血管病变患者.结果利用胶原酶灌注消化以及Ficoll分离液分层纯化技术的方法能够成功获得纯化的成人胰岛细胞;经胰岛素释放实验表明,实验中收获的胰岛具有良好的胰岛素分泌功能;经临床实践,该例糖尿病患者输注此胰岛后,血糖和糖化血红蛋白较移植前下降(P<0.05),腹泻症状较术前明显改善,体重增加了4.5kg,移植后无特殊并发症发生.结论成人胰岛细胞移植治疗I型糖尿病安全有效,此例为云南省成人胰岛细胞移植平台的建立和应用提供了理论和实验依据. Objective To treatment of diabetes. Methods study the the clinical value of human pancreatic islet cell transplantation in the Since 2006, we had mastered the technology of human pancreatic islet cell transplantation by repeated animal experiments and continuous exploration, and applied the technology to one case of type 1 diabetes with severe mesenteric vasculopathy. Results We obtained human pancreatic islet cell successfully by the method of pancreatic islet isolation and purification adopted in this research, and the insulin release experiments demonstrated that tile insulin secreting function of the islets was good. The infusion of the islets led to the descent of blood sugar and glycosylated hemoglobin (P 〈 0.05), and the improvement of diarrhea in the diabetes patient. After transplantation, the weight of this patient increased 4.5 kg, and no complications were found. Conclusion The method of human pancreatic islet cell transplantation is safe and effective, which provide theoretical and experimental basis for the platform of human pancreatic islet cell transplantation in our province.
