后部缺血性视神经病变27例分析

Analysis of 27 cases Posterior ischemic optic neuropathy

目的通过回顾性研究，探讨后部缺血性视神经病变（posteriorischemicopticneuropathy，PION）的临床表现、诊断及治疗预后。方法对27例（33只眼）PION患者进行常规眼部检查及全身检查，观察治疗前后视力、视野、眼前节及眼底体征变化。结果治疗前所有患者均有不同程度视力下降和视野缺损。治疗后有29只眼视力有不同程度提高，视野缺损也有改善，有4只眼视力无明显改变，有效率为87．9％。结论颈动脉供血不足是PION的重要致病因素，早期诊断和积极治疗有助于视力改善。

Objective To discuss the clinical manifestation, diagnosis, therapy and prognosis of posterior ischemic optic neuropathy （PION） via retrospective research. Methods To routinely perform examination of eyes and all systemically to 27 cases （33 eyes）, then observe changes of visual acuity, visual field, anterior eye segment and fundus. Results Decline of visual acuity and damage of visual field happened to different degrees in all patients before and after therapy, visual acuity of 29 eyes raised to different degrees, and visual field also progressed, 4 eyes had no obvious change on visual acuity, ratio of efficiency was 87.9%. Conclu- sions Shortage of carotid blood supply was the important pathogenic factor of PION, diagnosis and active therapy in the early stage benefited for improvement of visual acuity.
