细菌性肝脓肿前期区域灌注治疗的临床研究

Clinical research on treatment of early-stage bacterial liver abscess using regional perfusion therapy

目的:探讨区域灌注疗法对细菌性肝脓肿前期(急性期炎症感染尚未形成液化区)的治疗价值。方法:将56例未液化性肝脓肿患者随机分为两组,治疗组为全身应用抗生素加在CT或B超引导下经皮肝穿刺置管区域灌注敏感抗生素,对照组为单纯全身应用抗生素。分别观察患者肝区疼痛情况、体温、外周血白细胞及B超检查肝脏局部感染灶转归情况。结果:治疗组患者自觉症状消失快、住院时间短、住院费用低、无感染灶液化病例,两组比较,有显著性差异(P<0.01)。结论:细菌性肝脓肿前期全身加区域灌注抗生素的治疗方法是一种新方法,且操作简单、有效,患者住院时间明显缩短,费用降低,值得临床推广应用。

Objective： To explore the treatment of the early-stage bacterial liver abscess （Non-liquefaction Bacterial liver abscess） using regional perfusion therapy. Methods： 56 cases of non-liquefaction bacterial liver abscess patients were divided into two groups, the control group were systemly applicated with antibiotics simply, the treatment group were applicated regional perfusion antibiotics accroding to percutaneous liver puncture under CT or B-mode ultrasonography based on the control group. Respectively, to observe the pain, temperature, blood WBC and the transition of local liver infection under B-mode ultrasonography. Results： The self-symptoms of patients disappeared quickly, the patients had a short hospital stay and low cost, no infected liquescence cases, there was a significant difference between the two groups （P〈0.01）. Conclusion： The treatment method of early bacterial liver abscess in systemly applicating with antibiotics simply and regional perfusion antibiotics is new. It is so simple, effective, shorten the time of patients, and Lower costs that it is worth for clinic use.
