醛固酮在慢性肾脏纤维化中的作用及机制

摘要 肾小球硬化和肾小管间质纤维化是各种原因引起的终末期肾病的共同病理特征。醛固酮是肾脏疾病发展中一个独立的因素,由基因和非基因作用方式发挥作用,主要由上调转化生长因子-β1、结缔组织生长因子、表皮生长因子受体、骨桥蛋白、核因子-κB、活化蛋白等,增加反应氧自由基,引起血管平滑肌细胞功能异常,增强纤溶酶原激活物抑制剂作用等环节导致纤维化,而这些作用途径并非孤立的,它们之间是互相影响、互相关联的。醛固酮受体拮抗剂能够有效减轻蛋白尿、改善肾功能、延缓肾脏纤维化进程。
