摘要
卫生部制定的处方管理办法把用药安全的责任赋予了临床药师。其中第36条规定,药师发现严重不合理用药或者用药错误,应当拒绝调剂,并按照有关规定报告。然而,用药错误应该怎么报告,向谁报告?报告了之后应该怎么处理?
Provisions for Prescription Drug Safety promulgated by Ministry of Health entrusted the responsibility of medication safety to clinical pharmacist.According to Article 36 in the Provisions for Prescription Drug Safety,in the case that pharmacists fin d severe irrational clinical drug use or error in clinical drug use,pharmacists shall refuse to make up the prescription,and shall report to the physicians who prescribe the drug,record the incident,and report according to the relevant regulations.How to report medication error,and to whom the report shall be submitted? How to deal with medication error after reporting?
