侧裂池开放术治疗难治性硬膜下积液16例临床体会

Report of 16 cases with intractable subdural effusion treated by opening of the sylvian fissure

目的探讨经颅锁孔显微镜下侧裂池开放术治疗难治性硬膜下积液的临床疗效,并分析其作用机制。方法2004年1月～2008年7月对16例难治性硬膜下积液患者行经颅锁孔显微镜下侧裂池开放术,使积液腔与侧裂池相通。结果术后1～3个月复查头CT硬膜下积液完全消失14例,大部消失2例,积液无复发,相关临床症状及体征消失。结论经颅锁孔显微镜下侧裂池开放术治疗难治性硬膜下积液安全可靠,创伤小,效果满意。

[ Objective ] To study the therapeutic effect of microsurgery treatment For intractable subdural fluid collection treated by opening of the sylrian fissure and analyse its mechanism. [Methods] 16 patients from January 2004 to July 2007 with intractable subdural fluid collection treated by opening of the sylrian fissure. [Result] Subdural fluid collection disappeared totally in 14 case and subtotally in two case affter 1 -3 months, Relateef clinical symptoms also disappeared and not recurred. [ Conclusion] Microsurgery treatment for intractable subdural fluid collection treated by opening of the sylrian fissure is safe, minimally invasive and effective.
