预制鞘膜管治疗尿道下裂术后严重并发症(附11例报告)

Perform of vaginal tube for treating severe postoperative complications of hypospadias (11 cases)

目的探讨尿道下裂术后严重并发症的治疗原则及方法。方法总结11例应用预制鞘膜管治疗尿道下裂术后严重并发症患者的临床资料。结果11例患者中10例获得随访,并发尿道口狭窄、吻合口轻度狭窄各1例,经扩张后治愈;2例术后阴茎略显臃肿;其余均排尿通畅,无尿道瘘出现。结论治疗尿道下裂术后严重并发症,彻底切除阴茎腹侧瘢痕组织和全部病变尿道,充分伸直阴茎是手术成功的基础,应用一期预制的鞘膜管,带蒂转移至尿道床,二期重建尿道,手术后并发症少,治愈率高,是治疗尿道下裂术后严重并发症较好的方法。

[Objective] To explore the principle and method of treating severe eompheations of hypospadias. [Methods] A retrospective analysis was made of 11 patients with hypospadias underwent urethoplasty with performed vaginal tubes. [ Results ] 10 eases were followed up among the total 11 patients. Complications of 1 ease ure- throstenosis and 1 case anastomosis stricture were founded and both were cured by urethra dilatation, 2 eases bulky of penis and no other complications such as urethroeutaneous fistula were followed. [ Conclusions ] The succeeded treating of sever complications of hypospadia was based on completed scare tissue and urethra lesion cleaning and sufficient penile straightness. The tubes of tunica vaginalis flaps were performed and transplanted to urethra bed and then urethoplasty were made several months later. With these methods, patients have less complications and more healing rate and it＇s proved to be more ideal to deal with severe complications of hypospadias.
