摘要
目的研究复杂锁骨骨折缩短复位内固定的可行性。方法骨折类型采用Edward分型法。对13例碎裂严重、难以解剖复位的复杂粉碎性锁骨骨折,术中采取对骨折端有限缩短的方法,即把锁骨缩短5～10mm,常规使用锁骨钢板内固定,并将缩短后剩余的骨质植在骨折端。结果所有患者术后随访6～18月,骨折愈合良好,锁骨部位和肩关节外形功能正常。根据JOA肩关节评分系统评价疗效,优11例,良2例。结论对难以解剖复位的复杂粉碎性锁骨骨折,缩短复位内固定是一种可行的手术方法,可以简化手术操作,骨折端稳定性更好,有利于骨修复,不影响肩部的外形与功能。
Objective To study the possibility of comminuted clavicular fracture treated with shorten reduction and internal fixation. Methods Thirteen cases of comminuted clavicular fracture classified with Edward method were operated. Fracture was shorten for 5 - 10 ram. Reduction and internal fixation was performed. Remained bone was grafted. Results All patients were followed - up for 6 - 18 months. Bone union was found in all patients. According to JOA score system of shoulder function, 11 cases were excellent,2 cases were good. Conclusions Shorten reduction and internal fixation is a possible method for comminuted clavicular fracture. It benefits to simplified operation and bone union. Shoulder configure and function restore well.
