腹部开放伤合并海水浸泡致犬急性心力衰竭临床特征

Clinical characteristics of dogs after open abdominal injury and seawater immersion induced acute heart failure

目的:观察犬腹部开放性损伤经海水浸泡后致急性心力衰竭的临床特征.方法:以成年杂种犬为实验动物犬随机分为腹部伤+海水浸泡组(A组,n=8)和单纯腹部伤组(B组,n=8).观察实验动物致腹部开放性损伤前、后及经海水浸泡2h后打捞出水不同时间点心律、心率(HR)、收缩压、舒张压、平均动脉压(MAP)、肺动脉嵌压(PAWP)、中心静脉压(CVP)、心输出量(CO)、心脏指数(CI)、B型脑利钠肽(BNP)、心肌酶谱、氧分压等相关指标.并于致伤前及致伤后4h行超声心图,测定LVEDV,LVESV,SV,LVEF.结果:A组在浸泡后HR、收缩压、舒张压、MAP,CVP均下降.至2h时,PAWP>18mmHg(1mmHg=0.133kPa),CO减低伤前30%,CI,LVEDV,LVESV,SV,LVEF均较B组减低(P<0.05),肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、BNP均较B组显著升高(P<0.05).结论:犬腹部开放伤合并海水浸泡后出现急性心力衰竭的特征.

AIM：To observe the clinical characteristics of dogs after open abdominal injury and seawater immersion induced acute heart failure. METHODS：Adult mongrel dogs as experimental animals were randomly divided into open abdominal injury＋seawater immersion group （A group n=8） and simple open abdominal injury group（B group n=8）. To observe experimental animals caused by open abdominal injury before and after,and the seawater immersion after 2 h the water at different time points salvage cardiac rhythm,heart rate（HR）, systolic pressure,diastolic pressure, mean arterial pressure （ MAP ）, pulmonary wedge pressure（ PAWP）, central venous pressure（CVP）, cardiac output （ CO ）, cardiac index （ CI ）, B-type brain natriuretic peptide （ BNP）, cardiac enzymes, partial pressure of oxygen and other relevant dimensions. On the injury before and after injury 4 h had ultrasonic cardiogram to measure LVEDV, LVESV, SV, LVEF. RESULTS： A group in the post-immersion heart rate, systolic pressure, diastolic pressure, mean arterial pressure, central venous pressure decreased. To 2 h, the puhnonary artery wedge pressure 〉 18 mmHg, cardiac output to reduce the 30% pre-injury, cardiac index, LVEDV, LVESV, SV, LVEF than those in group B reduced （ P 〈 0.05 ）, creatine kinase （CK） , creatine kinase isoenzyme （CK-MB）, BNP than those in B group was significantly higher （ P 〈 0.05 ）. CONCLUSION ： The dogs after open abdominal injury with seawater immersion expressed the characteristics of acute heart failure.
