基于期刊文献研究神经根型颈椎病治疗方剂用药、配伍特点的探讨

Study on the Characteristics of Composition and Synergy of Prescriptions for Cervical Spondylotic Radicu lopathy based on Literature Reviews

目的：总结分析期刊神经根型颈椎病（CSR）治方用药特点和配伍规律。方法：检索、获取符合标准资料完整的CSR治疗方剂,建立方剂数据库;统计方剂的药物功效类别使用频数,提取方剂常用的药对（组）,比较内服方剂和外用方剂用药的差异。结果：485个方剂纳入研究。308个内服方剂以汤剂使用最多,以活血化瘀药、补虚药、解表药、祛风湿药等类别较为多用;提取了桂枝～威灵仙、当归～红花、当归～白芍、羌活～川芎、葛根～桂枝等不同方式配伍的药对（组）。177个外用方剂,以活血化瘀药、祛风湿药、解表药等类别使用居多;提取了川乌～草乌、乳香～没药、川芎～威灵仙等不同方式配伍的药对（组）。期刊内服、外用方剂的祛风湿、解表、补虚等5类功效类别、部分常用药物的使用频率的差异有统计学意义。结论：活血化瘀、补虚、祛风湿等类别药物是CSR治疗方剂主要组成药物,内服方剂多使用补益、祛风湿类药物,而外用方剂则多使用祛风寒湿、活血化瘀类药物。

Objective：To summarize and analyze the characteristics of composition and synergy of the prescriptions for cervical spondylotic radiculopathy（CSR） based on literature reviews.Methods：All the prescriptions used to treat CSR were retrieved and collected.According to different efficacies and application frequencies,the internal and external prescriptions of the herbal medicine were compared.Results：A total of 485 formulae were covered in this study.Within the 308 internal prescriptions, there were 31 herbs of blood--activating and stasis resolving drug used 829 times, 34 herbs of supplementing drug used 671 times, 17 herbs of relieving superficies drug used 637 times, and 37 herbs of wind-dampness dispelling drug used 493 times. While among the 177 external formulae, there were 36 herbs of blood-activating and stasis-resolving drug used 602 times, 36 herbs of wind-dampness dispelling drug used 562 times, 18 herbs of relieving superficies drug used 334 times, and 20 herbs of supplementing drug used 191 times. The above three main kinds of herbs and other frequently used herbs were differently applied in internal and external formulae. Conclusion： The prescriptions for CSR mainly comprise three kinds of Chinese herbs： the blood-activating and stasis-resolving drug, the supplementing deficiency drug and the wind-dampness dispelling drug. The internal prescriptions are largely composed of the supplementing drug and the winddampness dispelling and cold dispersing drug, while the external prescriptions mostly consist of the wind-dampness dispelling and cold dispersing drug and the blood-activating and stasis-resolving medicine.
