摘要
目的探讨海南黎族高血压并动脉硬化者血管紧张素转换酶(ACE)基因多态性与颈动脉粥样硬化的相关性。方法采用聚合酶链反应(PCR)检测260例海南黎族高血压并动脉硬化患者及276例黎族正常人的ACE基因插入/缺失(I/D)多态性,观察DD、DI、II基因型频率及等位基因频率。结果 (1)高血压并动脉硬化组DD、DI、II基因型频率为15.0%、37.3%、47.7%;D及I等位基因频率分别为33.7%、66.3%。正常对照组DD、DI、II基因型频率17.8%、40.6%、41.6%,D及I等位基因频率分别为38.0%、62.0%。两组之间DD、DI、II基因型频率及D、I等位基因频率无显著性差异(P>0.05)。(2)在所有高血压者中,ACE DD型的MIMT比DI和II型亚组显著增高(P<0.05)。在高血压1级和3级组内,ACE DD型亚组的MIMT比DI和II型亚组显著增高(P<0.05),但组间比较无统计学差异(P>0.05)。结论 ACE DD基因型增加了颈动脉硬化易感性,是海南黎族高血压并动脉硬化患者的危险因素,可作为动脉粥样硬化的一个早期预测因子,但DD型高血压并动脉硬化中颈动脉MIMT与高血压分级无正相关性。
Objective To study the correlation between angiotensin -converting enzyme (ACE) gene polymorphisms with hypertension with atheroselerosis and carotid atherosclerosis in Hainan Li Nationality. Methods ACE I/D gene polymorphisms were detected by polymerase chain reaction (PCR) in 260 eases of hypertension with atheroselerosis group and 276 eases of healthy group from Hainan Li nationality. The genotype frequencies and allele frequencies of DD, DI and Ⅱ were investigated. Results ( 1 ) In the hypertension with atheroselerosis group of Hainan Li nationality, the genotype frequencies of DD, DI and Ⅱ were 15.0% , 27.3% and 47.7% respectively, and the allele frequencies of D and Ⅰ were 33.7% and 66.3% respectively. In the healthy control of Hainan Li nationality, the genotype frequencies of DD, DI and Ⅱ were 17.8% , 40.6% and 41.6% respectively, and the allele frequencies of D and Ⅰ were 38.0% and 62.0% respectively. There were no significant differences both in the genotype frequencies of DD, DI and Ⅱ, and in allele frequencies D and Ⅰ between these two groups (P 〉 0.05). (2) In all hypertensive patients, the MIMT( mean intimal -medial thickness) level in ACE DD subset was significant higher than that in DI and Ⅱ(P 〈0.05). In the hypertensive grade 1 and grade 3 group, the MIMT level in ACE DD subset was significant higher than that in DI and Ⅱ(P 〈 0.05) respectively. There were no significant differences between the two groups (P 〉 0.05 ). Conclusion The ACE DD genotype increases the susceptibility of carotid atherosclerosis. It is the risk factor in the hypertension with atherosclerosis in Hainan Li nationality. It may be an early predictive factor in atherosclerosis. But the MIMT level of the DD genotype in the hypertension with atherosclerosis is not related with hypertensive classification positively.
