摘要 目的观察波长407~420nm蓝光对痤疮患者的疗效,对面部皮肤皮脂溢出率及糠秕马拉色菌的影响。方法对20例痤疮患者蓝光治疗前后行电子显微成像观察,评价靶疹的表皮层及真皮浅层变化;应用Looking bill及Cunliffe改良测定法进行皮脂溢出率测定;应用蓝光对培养的糠秕马拉色菌进行照射,观察照光前后菌落的数量及形态变化。结果20例痤疮患者蓝光治疗后靶疹明显消退,其表皮层及真皮浅层损伤减轻;皮脂溢出率较治疗前显著减少(P<0.01);在0.5ml细胞数为5×106/ml的糠秕马拉色菌菌液中,菌落数量、直径与光照时间成反比,蓝光光照时间大于3min30s时,平皿内无菌落生长。结论蓝光对痤疮有很好的治疗效果:能修复深至真皮层的皮肤损伤,抑制皮脂分泌,对糠秕马拉色菌有杀菌作用。 Objective To investigate the effects of blue light(407 to 420 nm) therapy on acne,facial sebum excretion rate and the growth of Malassezia furfur isolated from acne lesions.Methods Twenty cases of acne patients were treated with the blue light system.Before and after the treatments,electronic microscopy were used to assess the change of the target acne in epidermis and dermis.Looking bill and Cunliffe s technique was used to test the sebum excretion rate(SER).Selected bacterial strains of Malassezia furfur were radiated with the blue light. The morphology and number of colony formation units (CFUs) of the bacteria were observed before and after the irradiation. Results Twenty cases of acne patients' target acnes fade away remarkably and the injuries in the epidermis and dermis were alleviated. The SER was decreased significantly (P 〈 0. 01 ) after the treatment of blue light. There was no coenobium of Malassezia fuoCur when the time of blue light irradiation exceeded 3.5 min. The number and diameter of the coenobium were in inverse proportion to the irradiation time. Conclusion The blue light can treat acne effectively, restore the damage even in the dermis, restrain the sebum secretion and kill the Malassezia furfur. It provides the experimental evidences for the treatment of acne.
