摘要
目的:探讨不同临床类型和病理类型的腺性膀胱炎与癌前病变的关系。方法:对40例腺性膀胱炎进行临床分型和病理分型,10例正常膀胱黏膜组织作为对照,采用免疫组织化学SABC法检测不同类型腺性膀胱炎和正常膀胱黏膜组织P53和Ki67的表达情况,并利用全自动显微镜及图像分析系统对免疫组化染色结果进行图像采集及分析。结果:临床分型中,乳头型和肠腺瘤型腺性膀胱炎P53和Ki67阳性表达明显高于慢性炎症型和滤泡型腺性膀胱炎(P<0.05)。病理分型中,肠上皮型和前列腺上皮型腺性膀胱炎P53和Ki67阳性表达与移行上皮型比较差异有统计学意义。结论:临床分型中,乳头型和肠腺瘤型腺性膀胱炎可能为癌前病变。病理分型中,肠上皮型和前列腺上皮型腺性膀胱炎可能为癌前病变。因此腺性膀胱炎是否具有恶变潜能应将二者相结合综合考虑,乳头型和肠腺瘤型腺性膀胱炎若伴有肠上皮化生或前列腺上皮化生则发生癌变的概率更大。
Objective:To study the relationship between the different clinical type and pathological type of cystitis glandularis and precancerous lesion. Methods:The different clinical type and pathological type of 40 cases with glandularis cystitis were performed, 10 cases of normal bladder mucosa tissue as control group. The expression of Ki67 and P53 in the cases of different types of cystitis glandularis was detected by using immunohistochemistry SABC method. At the same time, the results of expression were analyzed by the Computer Image Analysis System. Results:In the clinical types, the positive expression of Ki67 and P53 of nipple type and intestinal adenoma type was significantly higher than the chronic inflammatory type and the follicular type(P〈0.05). In the pathological types, the positive expression rate of P53 and Ki67 of the intestinal epithelial type of cystitis glandularis and the prostate epithelial type of cystitis glandularis is significantly higher than the transition epithelial type. There is statistical significance. Conclusions.. In the clinical type, the nipple type and the intestinal type of cystitis glandularis are precancerous lesion. In the pathological types, the intestinal epithelial type and the prostate epithelial type of cystitis glandularis are precancerous lesion. It need consider the upper two sides together to deduce that the cystitis glandularis is precancerous lesion or not. If the nipple type and the intestinal adenoma type of cystitis glandularis have intestinal or prostate epithelial metaplasia, the rate of its malignant change is very higher.
