摘要
目的观察高血压和冠心病患者是否存在红细胞分布宽度(RDW)的异常。方法选择南昌大学第二附属医院2007-06-2008-06经冠脉造影确诊为冠心病的患者287名,确诊为原发性高血压(无冠心病)的患者286名,并选取同期门诊体检正常的成人302名作为对照组。采受试者清晨空腹静脉血,用全自动血细胞分析仪测定红细胞数、血红蛋白、RDW。比较各组患者RDW的差异。结果高血压组与正常对照组之间RDW无明显差异[(12.9±0.6)%比(13.0±0.7)%,P>0.05],但冠心病组RDW[(13.2±0.8)%]则明显高于高血压组和正常对照组(均P<0.05)。性别分析发现,3组男性间RDW无明显差异,而女性冠心病组RDW则明显高于高血压组和正常对照组[冠心病组(13.5±0.8)%比高血压组(13.0±0.6)%,对照组(13.1±0.8)%,均P<0.05]。无论男女,高血压组与正常对照组之间无明显差异;女性冠心病组RDW明显高于男性冠心病组[(13.5±0.8)%比(13.1±0.7)%,P<0.05]。相关分析显示,冠心病组RDW与血红蛋白呈弱负相关(r=-0.22,P<0.01)。结论无论男女,高血压患者RDW无明显改变;冠心病组RDW增大,主要为女性患者明显增大而男性无明显改变。
Objective To observe the changes of red blood cell distribution width (RDW) in patients with cor- onary heart disease (CHD) or hypertension. Methods Two hundred and eighty-seven CHD patients diagnosed by coronary ang.iography and 286 hypertensive patients were selected from Jun. 2007 to Jun. 2008. Three hundred and two healthy adults were enrolled as control group. Red blood cell count, hemoglobin and RDW were detected by full automatic hemocyte analyzer. Results No difference in RDW between hypertension patients and normoten- sives [{12.9±0.6)% vs (13.0±0.7)%, P〉0.05], while the RDW in CHD patients was significantly increased I-(13.2±0.8)%, both P〈0.05]. Gender analysis found that there was no differences of the male RDW among three groups. However, the RDW of female CHD group was higher than that of female hypertension and normal groups[- (13.5±0.8)% vs (13.0±0.6)% vs (13.1±0.8)%, both P〈0. 05]. TheRDWoffemaleCHDpatients was higher than that of male CHD patients [-(13.5±0.8)% vs (13.71±0.7)%, P〈0.05]. Correlation analysis showed RDW was weakly, and negatively associated with hemoglobin (r=- 0.22, P〈0.01) in CHD group. Conclusion The RDW was in normal range in hypertensive patients, while the RDW in CHD patients especially in female was significantly increased.
