直肠全系膜切除术治疗低位直肠癌38例临床体会

Clinical Experience on the Treatment of Low Rectal Carcinoma of 38 Patients by Total Mesorectai Excision

为探讨低位直肠癌保肛手术治疗的效果，对2001年1月至2005年12月我院采用直肠全系膜切除术（TME）治疗的38例低位直肠癌患者病例资料进行回顾性分析。结果显示，术后吻合口狭窄2例（5．2％），经扩肛后患者能正常排便；术后吻合口漏1例（2．6％），经保守治疗痊愈。随访至2008年12月，肿瘤局部复发1例，为低分化腺癌。结果表明，低位直肠癌保肛手术是安全可行的。

In order to investigate the efficacy of anus-saving operation for low rectal carcinoma authors analysed the data of 38 patients with low rectal carcinoma who had been subjected to total mesorectal excision （TME） from 2001-01 to 2005-12 retrospectively. As results, postoperative stenosis of anastomotic stoma occured in 2 cases（5.2 % ）, via dilation the 2 patients had normal defecation; postoperative anastomotic leakage occured in one case（2.6%） ,then cured by conservative treatment. The follow-up to 2008-12 found local recurrence for tumor of one case being poorly differentiated adenoearcinoma. In a word, anus-saving by TME for low rectal carcinoma is safe and feasible.
