3种不同用药方案治疗社区2型糖尿病的成本-效果分析

Three Different Drugs in the Treatment of Community-based Type 2 Diabetes Cost-effectiveness Analysis

目的评价3种不同用药方案治疗2型糖尿病的疗效及经济成本。方法选择我院180例2型糖尿病患者,随机分为3组,实施3种联合用药治疗方案,12个月后进行成本-效果分析。3种不同方案为:二甲双胍+格列本脲(A组)、二甲双胍+格列齐特(B组)和二甲双胍+格列吡嗪(C组)。结果A、B、C3组方案药物成本分别为434.28元、910.60元和813.91元;对空腹血糖的总有效率分别为85.0%、90.0%和88.3%;对餐后2h血糖总有效率分别为83.3%、88.3%和86.7%;空腹血糖的成本-效果比分别为2.53、7.68和6.74;餐后2h血糖的成本-效果比分别为2.58、7.83和6.86;B、C2组相对于A组对空腹血糖的增量成本比分别为95.30、140.60。B、C两组相对于A组对餐后2h血糖的增量成本比为95.00、237.20。结论3组治疗2型糖尿病的方案疗效均较好。从成本-效果比分析,二甲双胍+格列本脲方案更符合药物经济学原则。

OBJECTIVE To evaluate the 3 different drugs in the treatment of type 2 diabetes and economic costs. METHODS 180 cases of hospital patients with type 2 diabetes were randomly divided into 3 groups to carry out three kinds of combination treatment, 12 months after the cost - effectiveness analysis. Three kinds of different options as follows： mefformin ＋ glibenclamide（A group） , metformin ＋ gliclazide（B group） , mefformin ＋ glipizide（C）. RESULTS A, B, C 3 group program of drug costs were 434.28 yuan, 910.60 yuan and 813.91 yuan; of fasting blood glucose of the total effective rates were 85.0%, 90.0% and 88.3%; 2h postprandial blood sugar of the total effective rates were 83.3%, 88.3%, 86.7%; fasting the cost of blood sugar - than were 2.53,7.68,6.74; 2 h postprandial blood sugar cost - than were 2.58,7.83,6.86; B, C group compared with the A group of fasting blood glucose than the incremental cost of 95.30, 140.60. B, C group compared with the A group 2 h postprandial blood glucose ratio of the incremental cost 95.00, 23%20. CONCLUSION The three groups of treatment of type 2 diabetes have better efficacy of the program. From the cost - than analysis, mefformin ＋ glibenclamide program more in line with the principles of pharmacoeconomics.
