间歇光疗法治疗新生儿高未结合胆红素血症50例疗效观察

The Clinic Effect of Intermittent Phototherapy of Neonatal Unconjugated Hyperbilirubinemia

目的探讨间歇光疗对新生儿高未结合胆红素血症的效果及副作用。方法随机将100例高未结合胆红素血症患儿分成实验组和对照组各50例。所有病例均给予酶诱导剂及白蛋白综合治疗的基础治疗。在此基础上实验组施行间歇光疗分次疗法,光疗时间4～8h/d,2～4h/次,2次/d,疗程3～5d。对照组持续光疗时间12h/d,1次/d,疗程3～5d。分别予以不同的光疗时间,比较两组光疗效果及副作用。结果:实验组光疗后总胆红素、间接胆红素和直接胆红素下降值均明显高于对照组(P<0.05);光疗后实验组血清游离脂肪酸C160、C180和C181含量均明显低于对照组,差别具有统计学意义(P<0.05);实验组总光疗时间、副作用恢复时间及黄疸消退时间均明显低于对照组,差别具有统计学意义(P<0.05)。结论:间歇光疗有助于血清胆红素水平下降,副作用少,达到了预期效果。

OBJECTIVE To investigate the clinic effects and side effects of intermittent phototherapy of neonatal unconjugated hyperbilirubinemia. METHOD 100 cases unconjugated hyperbilirubinemia in children were randomly divided into experimental and control groups.All patients were given the basis ＇treatment of albumin enzyme combined with inducing agents treatment.On the basis of this,the experimental group was given the intermittent phototherapy fractionated therapy,light therapy time 4-8h/d, 2~4h/times, 2times/d, treatment 3~5d. Control group was given the continuous phototherapy, 12 h/d, 1 times/d,treatme＇nt 3-5d.After the phototherapy time, compare the effects and side effects. RESULTS total bilirubin after phototherapy,indireet bilirubin and direct bilirubin were significantly higher in experimental group than those in control group（P〈0.05）; the the serum ferr fatty acids C160,C180 and C181 were significantly lower content in experimental group than that in the control group,with statistically significant differences （P〈0.05） ; the recovery time and side-effects subside time jaundice in the experimental group was significantly lower than that in the control group,the difference was significant （P〈O.O5）.CONCLUSION Intermittent light therapy will help decrease the level of serum bilirubin,less side- effects to achieve the desired effect.
