后路TFC治疗退行性腰椎病变术后远期疗效分析

Posterior Lumber Interbody Fusion Using TFC for Degenerated Lumber Disease:A Long-term Efficacy Study

摘要 目的分析单纯TFC用于后路腰椎融合治疗退行性腰椎病变术后远期的疗效。方法自1997年1月~2000年6月共以TFC行腰椎后路椎间融合术(PILF)治疗腰椎退行性病变35例。腰椎间盘突出症23例,腰椎滑脱9例,单纯腰椎管狭窄3例。观察患者的远期临床效果及影像学结果。结果随访8~12年,根据JOA临床改善率标准,术后远期疗效的优良率达94.3%(33/35);影像学上3例融合器小幅度下陷,有1例融合器轻度向后移位,但均未出现相应的临床症状;融合临近节段病变的发生率为22.9%(8/35)。结论单纯TFC行PILF治疗腰椎退行性病变具有良好的术后远期疗效,是一种简单、有效、经济的手术方法,但应强调手术的适应证。 Objective To evaluate long-term clinic outcome of TFC in posterior lumber interbody fusion for degenerated lumber disease. Methods From 1997 to 2000, 52 consecutive patients with single segment degenerated lumber disease were included in this study.Thirty-five patients were followed up, including 14 males and 21 females with average age of 45.9 years old (range 27-58 years). All of the patients underwent posterior lumber interbody fusion with TFC and the long-term clinic outcome was observed. Results Thirty-five patients were followed up for 8-12 years. According to JOA criterion, the excellent and good rate was 94.3% and the bone graft fusion rate was 100%.From the X-ray 3 cases appeared subsidence and 1 case appeared posterior displacement slightly without complaint.Incidence of adjacent segment disease was 22.8%.Conclusion As a simple, effective operation, posterior lumber interbody fusion using TFC has a satisfactory long-term efficacy for degenerated lumber disease.
