椎间融合器结合不同固定法治疗椎间盘源性腰椎不稳症的比较研究 被引量:2

摘要 目的比较椎间融合器结合不同固定法治疗椎间盘源性腰椎不稳症的效果。方法将102例椎间盘源性腰椎不稳症根据治疗方法不同分为三组:聚醚醚酮椎间融合器(PEEK)加关节突螺钉固定组(A组),椎间融合器加椎弓根钉固定组(B组),单纯椎间融合器组(C组),三组均行椎间植骨。结果三组术后较术前各项指标有明显改善(P<0.01)。随访发现:C组椎间隙高度、椎间孔高度丢失程度较A组、B组明显(P<0.05);B组术中出血较多、C组融合器后移及后期融合器下沉发生率较高;JOA评分A组优良率为93%,优于其他两组(P<0.05)。结论PEEK椎间融合器加关节突螺钉固定治疗椎间盘源性腰椎不稳症临床疗效肯定,操作相对简便、创伤小、大大减少了患者的费用。术后还能保持较好的腰椎活动度,并发症相对较少。
