血液维生素C浓度干扰尿酸检测准确性的限值测定

摘要 目的结合维生素C对尿酸酶-过氧化物酶偶联法测定尿酸的负干扰,介绍在理想状态下即不联合应用其它还原性药物时,该试剂测定尿酸值在临床可接受最大干扰范围时的维生素C浓度检测方法。方法在同一份标本中加入不同浓度维生素C后测定尿酸,寻找Roche(含抗坏血酸氧化酶)和金斯尔(不含抗坏血酸氧化酶)两种试剂抵抗维生素C干扰的维生素C浓度临床可接受值和临床最大允许值。结果金斯尔试剂在维生素C浓度<0.022mmol/L时,Roche试剂在维生素C浓度<2.84mmol/L以下时所产生的干扰在临床可接受范围内,维生素C浓度达到5.68mmol/L时,干扰已超出临床最大允许范围。结论尿酸是反应肾脏功能的重要指标,尿酸浓度变化与某些疾病的相关关系越来越受临床重视。建议使用含有抗坏血酸氧化酶的尿酸试剂,尽量减少因使用维生素C等还原性药物所致的假性低尿酸血症。 Objective On the basis of Vitamin C that interferes uricase-peroxidase measuring in negative,to introduce the meathod of detecting Vitamin C concentration which interfere uric acid value of some reagent in the largest clinical acceptable range in an ideal state, i.e, do not unite other reducing drug.Method Mixing different concentration of VitaminC in the same sample and then measuring uric acid value, to find the concentrations of the two reagent ROCH(with ascorbate oxidase) and Gcel (without ascorbate oxidase) which could make the uric acid value in the clinical acceptable range and the maximum allowable. Result GCEL reagent could make the interference in the clinical aceeptabie range when Vitamin C concentration is less than 0.022mmol/L,ROCH reagent is less than 2.84mmol/L.When Vitamin C concentration reaches 5.68mmol/L, the interference is out the maximum allowable range. Conclusion Uric acid is the important guide line of reflecting kidney function. Authors advice that laboratories should use reagent with aseorbate oxidase to avoid pseudo hyporuricemia.
