摘要
目的探讨慢性阻塞性肺疾病(COPD)合并肺间质纤维化(PIF)时早期CT征象与肺功能各分级之间的对应关系。资料与方法应用高分辨率CT(HRCT)对50例COPD患者进行检查,选择8种常见的PIT CT征象,分别是小叶间隔增厚、肺内异常线影、磨玻璃样影、蜂窝状影、结节影、胸膜下线、细支气管扩张、支气管血管束增粗。以肺功能分级为参照,研究对应的CT特征,分析COPD合并PIF时,肺功能各分级所对应的CT征象。结果在8种CT征象中,小叶间隔增生、肺内异常线影、结节影、支气管血管增粗有随肺功能分级从低到高增加的趋势。结论COPD患者在肺功能分级为1和2级时绝大部分已经开始出现PIF,CT征象为小叶间隔增粗和肺内异常线影,而且这两种征象有相关性。
Objective To study the relationship between the early CT features and lung function grading in patients of COPD combined with PIF. Materials and Methods Fifty COPD sufferers combined with PIF were checked up by HRCT. Eight kinds of common CT features included interlobular septal, abnormal lines, ground-glass shadow, honeycomb shadow,nodules, pleural line, bronchiole expansion and bronchial vascular thickening were chosen to analyze. Results Four kinds of CT features included interlobular septal, abnormal lines, nodule, and bronchial vascular thickening were increased with lung function grading from low to high level. Conclusion Most COPD made progress to PIF when lung function was at one and two grading. The CT f eatures were interlobular septal thickening and abnormal lines and there were correlations between them.
