摘要
目的:探讨缺血性脑血管病患者苍白球铁沉积的MR征象。方法:对968例缺血性脑血管病患者脑MRI进行回顾性研究,对苍白球铁沉积信号进行观察同时分析其患病的相关因素。结果:MRI梯度回波序列有助于早期、方便无创地观测到脑组织内铁沉积现象,苍白球铁沉积的危险因素为年龄、血胆固醇浓度、椎动脉狭窄程度。结论:金属铁在脑组织内可能存在"小脑-脑干-基底节(苍白球)"这一代谢通路,年龄的增长,血胆固醇代谢障碍及椎动脉逐渐狭窄引起脑干小脑等部位供血不足,使该铁质转运通路受损,铁无法正常排泄,沉积于内侧苍白球区域。
Objective:To investigate the risk factors causing globus pallidus iron deposition in patients with ischemie eerebrovaseular disease. Methods: The brain MRI images from 968 isehemie cerebrovaseular patients were studied retrospectively, iron deposition in glohus pallidus was observed,at the same time, analysis was performed with SPSS. Results: MILl gradient-echo sequence helped to observe the iron deposition phenomenon in brain tissue early, non-invasively and easily. The risk factors of iron deposition in Globus pallidus were age, serumcholesterol concentration, and vertebral artery stenosis. Conclusion:Iron in the brain may have the metabolic pathway of "cerehellum-brain stem-basal ganglia (Globus pallidus)". Age, metabolism disorder of cholesterol and vertebral artery stenosis cause brain stem-cerebellum isehemie, which damage the access for iron metabolizability, thereby leading to iron deposits in the medial globus pallidus.
