经前路病灶清除一期椎体间植骨内固定治疗胸椎结核

One-stage anterior surgeries by focal debridement,bone grafting and internal fixation in the treatment of thoracic vertebral tuberculosis

目的探讨经前路病灶清除一期椎体间植骨内固定治疗胸椎结核的疗效。方法手术治疗胸椎结核12例,经前路病灶清除一期椎体间植骨内固定。结果术后随访10～42个月,平均21.5个月,植骨全部骨性融合,后凸畸形校正满意,神经症状者得到改善,无感染及窦道形成。结论经前路彻底清除病灶一期椎体间植骨内固定治疗胸椎结核,病灶清除彻底,植髂骨可靠,内固定稳定,植骨融合率大大提高。

Objective To explore the clinical effects of one - stage anterior surgeries by focal debridement, hone grafting and internal fixation in the treatment of thoracic vertebral tuberculosis. Methods 12 cases of thoracic vertebral tuberculosis were treated with surgeries. All the cases were treated with one - stage anterior surgeries by focal debridement, bone grafting and internal fixation. Results Follow - up ranging from 10 to 42 months （ mean = 21.5 months） revealed that all cases had good osseous fusion and satisfactory correction of posterior process deformity. Patients who had had neurological symptoms had improvement. No infection or sinus formation occurred. Conclusion One - stage anterior surgery in the treatment of thoracic vertebral tuberculosis can completely clear the focal lesions. It has good fusion, safe and reliable internal fixation and significantly increased rate fusion of bone transplant.
