儿童过敏性紫癜肾脏损害危险因素的多因素分析

Renal damage in Henoch-Schonlein purpura:a multivariate analysis of the Risk factors

目的:探讨儿童过敏性紫癜(HSP)肾脏损害的临床危险因素。方法:回顾性调查140例住院并确诊的过敏性紫癜患儿的资料,包括年龄、性别、临床表现、实验室检查和于外院给予的治疗,对这些资料进行logistic回归多因素分析。结果:51例患儿发现有肾脏损害(36.4%),logistic回归分析表明导致肾脏损害的危险因素是胃肠道出血和反复性皮肤紫癜(P<0.05)。结论:胃肠道出血和反复性紫癜是过敏性紫癜导致肾脏损害的关键因素。因此,出现这两种表现的患儿应严密观察随访。

Objectives： To study the risk factors of renal damage in children with Henoch-Schonlein purpura （HSP）. Methods ： 140 cases wit h HSP were retrospectively studied, included ： sex, age, clinical manifestations, laboratory tests and treatment given in other hospital. Patients were assessed by logistic multivariate analysis. Results ：Evidence of renal damage was detected in 51 patients （36%）. Logistic regression analysis found that purpura relapse and gastrointestinal bleeding were the risk factors of the renal damage. Conclusion：Purpura relapse, and gastrointestinal bleeding were the risk factors of the renal damage. Patients with these clinica lmanifestations should be rigorously monitored.
