补气健脾中药对抗结核药物所致肝损害的预防作用

Protection of Herbal Medicines to Tonify Qi and Strengthen the Spleen on Antituberculotic Liver Damage

目的根据中医"扶土抑木"理论,探讨补气健脾中药对抗结核药物所致肝损害的预防作用。方法100例肺结核患者随机分为两组,每组50例,均在抗结核药物治疗的基础上口服凯西莱片护肝,治疗组加服补气健脾中药,两组疗程均为4周,观察两组患者药物性肝损害的发生率及药物性肝损害患者治疗前后的肝功能,包括血清丙氨酸氨基转移酶(ALT)、门冬氨酸氨基转移酶(AST)及总胆红素(TBiL)的变化。结果治疗组药物性肝损害发生率明显低于对照组(P<0.05)。两组药物性肝损害患者ALT、AST均明显升高(P<0.05或P<0.01),治疗组TBiL异常例数及严重程度均低于对照组,但由于例数少,无法进行统计学比较。结论补气健脾中药可降低药物性肝损害的发生率。

Objective To explore the protective effect of herbal medicine to tonify qi and strengthen the spleen on liver damage in- duced by antituberculotic according to the TCM theory ＂reinforce the earth to reduce the wood＂. Methods The 100 tuberculosis patients randomized into two groups, 50 in each, all took antituberculosis drugs with the liver-protecting medicine, Tiopronin, and the herbal medicines to nourish qi and strengthen the spleen were administered to those in the treatment group. Incidence of liver damage and the changes of liver function including serum alanine aminotransferase （ALT）, aspartate aminotransferase （AST） and total biliru- bin （TBiL） were observed for four weeks. Results In the treatment group, the incidence of liver damage was significantly lower than that in the control group （P〈0.05）. ALT and AST in both groups were significantly higher （P〈0.05 or ＇〈0.01）. The abnormal TBiL cases and their severity of the treatment group were less than those in the control group, but there was no enough cases for statistical comparison. Conclusion The treatment with herbal medicines to tonify qi and strengthen the spleen reduces the incidence of liv er damage.
