白内障手术切口大小对角膜内皮的影响

Influence to corneal endothelium of diffrent length incision in cataract surgery.

为了解白内障手术切口大小与术后角膜内皮变化的关系。方法：应用非接触式角膜内皮显微镜对三组不同大小切口的白内障术后角膜内皮计数和形态学变化进行观察，它们的切口分别为第一组（大切口）１０～１２ｍｍ，第二组（中切口）５．５～６．５ｍｍ，第三组（小切口）３．２ｍｍ，结果：三组术后角膜内皮损失率分别是：第一组１４．６５±８．１９％，第二组１４．５５±９．６３％，第三组１２．６５±９．７２％，三者无显著性差异（ｘ２检验，Ｐ＞０．０５）；三组术后形态变化相同：早期表现内皮细胞脱落和肿胀、伸长变形修复，三月以上形态改变趋于稳定。结论：白内障手术切口大小与角膜细胞内皮变化无直接关系，内皮的变化主要与操作技巧和术后前房反应的轻重有关。

Objective:To investigate the relation between different incision in cataract surgery and changes of corneal endothelium.Method:Observing the changes of corneal endothelium in 3 groups(different incision),The dicision was separately 10～12mm,5 5～6 5mm,3 2mm,Results:The rate of corneal endothelium loss was 14 65±8 19% in incision of 10～12mm,14 55±9 63% in incision of 5 5～6 5mm,12 65±9 72% in incision of 3 2mm,there wasnt obvious difference (x 2test,P>0 05).The rehabilitation of corneal endcthelum was the same in 3 groups.Conclusion:There was direct relation between the incision and correal endothelium,Changes in corneal endothelium are mainly relation to techniques and reflect in anterior uveitis.
