小梁切除术中应用不同浓度丝裂霉素的比较

探讨小梁切除术中应用不同浓度丝裂霉素（ＭＭＣ）的作用。４８例（６２眼）ＰＯＡＧ患者随机配对分为两组，一组在常规小梁切除术中辅助用０．１ｍｇ／ｍｌ丝裂霉素，另组在术中使用０．２ｍｇ／ｍｌ丝裂霉素，术后随访一年。两组术后眼压均有下降，与术前眼压比较，差异有高度显著性（Ｐ＜０．０１），而两组间术后眼压比较，差异无显著性（Ｐ＞０．０５）。术后并发症最多见为晶体混浊加重，且０．２ｍｇ／ｍｌＭＭＣ组发生率明显高于０．１ｍｇ／ｍｌ组，差异有高度显著性（Ｐ＜０．０１）。不同浓度ＭＭＣ辅助用于小梁切除术中，均能提高手术成功率，但应用低浓度ＭＭＣ能减少术后晶体混浊的发生。

Abstract We performed a 1 year study evaluating a dose response of the concentration of mitomycin in trabeculectomy surgery.Forty eight POAG patients(62 eyes)were randomized into two groups.One group was used mitomycin,0 2mg/ml,the other group used mitomycin,0 1mg/ml.All of the patients had successful decrease in IOP 1 year after surgery than before surgery (p<0 01).There was no significant difference between the IOP of two groups (p<0 05).Progressive lens opacification was the most frequent complication in the surgery.The incidence of progressive lens charges markedly increased in subjects receiving 0 2mg/ml mitomycin therapy group(p<0 01).Our results suggest that using different concentrations of mitomycin can increase successul rate of the trabeculectomy surgery,but lower concentration of mitomycin can decrease the incidence of progressive lens opacification.
