2型糖尿病患者小切口白内障囊外摘除及人工晶体植入术的临床观察

Clinical Observation of the Treatment of Small Incision Extracapsular Cataract Extraction Combined with Intraocular Lens Implantation for Cataract with TypeⅡ Diabetes Mellitus

目的观察2型糖尿病患者行小切口白内障囊外摘除及人工晶体植入术的临床效果。方法对2005年3月～2007年12月2型糖尿病者白内障57例(62眼)与同期年龄、晶状体核硬度相匹配的非糖尿病者白内障64例(76眼)行小切口白内障囊外摘除及人工晶体植入术,对术后视力及早期并发症进行分析。结果术后随访4周,两组视力均明显提高,最佳视力≥0.5者两组间的差异无统计学意义(χ2=0.50,P>0.05)。糖尿病组术后角膜病变、葡萄膜炎、虹膜脱色素明显高于非糖尿病组,两组间的差异有统计学意义(χ2分别为10.25,8.17,17.31,P<0.05);前房积血、暂时性高眼压对比组间的差异无统计学意义(χ2分别为0.44,0.04,P>0.05)。结论糖尿病组术后早期并发症较非糖尿病组发生率高,经过处理于2～4周恢复正常,对术后视力影响不大。糖尿病患者行小切口白内障囊外摘除及人工晶体植入术安全有效。

Objective To observe the clinical effects of small incision extracapsular cataract extraction combined with intraocular lens implantation for cataract with type Ⅱ diabetes mellitue. Methods 57 cataract cases（62 eyes）with diabetes and 64（76 eyes）cataract cases without diabetes were undergone small incision xtracapsular cataract extraction combined with intraocular lens implantation, visualacuity and complications after surgery were analyzed. Results Four week later after the operation, the vision of two groups were markedly improved .the visualacuity（ ≥0.5 ） was not significantly different between two groups（P〉0.05 ）. Complications such as keratopathy,uveitis,iridial decoloration in diabetics were significantly higher than that incases without diabetes（P〈0.05 ）. Hyphema, temporal high intraocular pressure were not significantly different between two groups（ P〉0.05 ）. Conclusion It is safe and effective to undergo small incision extracapsular cataract extraction combined with intraocular lens implantation for cataract with type Ⅱ diabetes mellitue.
