无痛胃镜的临床应用体会

Experience on clinical application of painless gastroscopy

目的:应用无痛胃镜术,以使患者能在安全、舒适、无痛苦状态下进行胃镜检查和治疗。方法:术前准备除按常规胃镜检查外,应配备多功能监护仪、氧气瓶、急救药品及气管插管等抢救设备,建立静脉通道。检查前5～10min口服2%的利多卡因胶浆后,常规鼻导管吸氧,并监测血氧饱和度和血压、心率。开放静脉通道,以40mg/10s的速度静脉注射丙泊酚,同时观察患者的反应调整给药速度,直至临床体征表明麻醉起效,待患者入睡后立即行胃镜检查。术中术后严密观察血氧饱和度。结果:1248例患者中,只有10例患者出现血氧饱和度轻度下降,给予高流量吸氧好转,并顺利完成检查。其余患者未发生严重的副反应。无痛胃镜术后一般5～10min清醒,受检患者100%满意。结论:无痛胃镜检查是一种安全、有效、舒适的技术。

Objective： To carry out gastroscopic examination and treatmentunder the safe and comfortable and painless by using painless gastroseopy for patients. Methods： Besides the preoperotive preparation in the normal gastroseopy, some rescue equipments were been prepared, such as muhifunetion monitor, oxygen bomb, first-aid medicine and trachea cannula, and established the vein passage at the same time. All patients were asked to drink 2% lidocaine plasmagel 5-10 rain before examination, and routine nasal tube of oxygen inhalation, monitor blood oxygen saturation and blood pressure and heart rate. Then, the Propofol Intravenous injection with 40 rag/10 s, the speed of administration were been adjusted on the basis of reaction of patients. To carry out gastroscopy after satisfactory anesthesia, the change of blood oxygen saturation be careful observed in operation and postoperation. Results： Only 10 patients were slight decreased of blood oxygen saturation in 1248 patients, and got better by high flux inhalation of oxygen, and gastroseopies were performed successfully. The others hadn＇t seriously subsidiary reaction. All patients could awaked in 5-10 min after painless gastroseopy and the degree of satisfaction were 100%.Conclusion： The painless gastroseopy is a technology of safe and comfortable and good for patient.
